Application forms

You can get forms from a police station. The example application form used here is provided by the Metropolitan Police Department, “Keishicho” (警視庁). You can download this application from here

If you get application forms from a police station, you go to a police station that takes charge of the area, where your car park is located.

Inkan (印鑑)

You bring “Mitomein stamp” (認印). If you don’t have Inkan, you need to order a Inkan engraved with your name.

Address

When you write your address on the forms, you just write it as a resident certificate says as much as possible. If you worry about writing the format of address, you go and have your resident certificate issued at a municipal office and refer to it when you write address on a column of an application form.

Length, width, height

when you refer to data about length, width, and height of Vehicle from a catalog or something, values smaller than millimeter should be omitted.

Vehicle Name (Shamei: 車名)

Car name is equivalent to the name of the manufacture, which is not unique car name. For example, you write トヨタ, even if it is named “Toyota Purius”.

Vehicle Model (Katashiki: 型式)

Viechle model is expressed with combination of number and alphabet.

Vehicle Identification Number (Shataibangou: 車台番号)

Vehicle Identification Number is a unique ID number engraved on a car platform.

You can find it with vehicle inspection certificate or by directly seeing the number engraved on a car platform.

The number is sometimes called “flame number(フレームナンバー)” or “manufacture number (Seizoubangou: 製造番号)”

Vehicle Size (Jidousha no ookisa: 自動車の大きさ)

Length (Nagasa: 長さ)

Width (Haba: 幅)

Your apartment address (Jidousha shiyou no Honkyono ichi: 自動車の大きさ)

More accurately, you wirte your address which is mentioned under the resident certificate

If you rent a car park in the name of your company, you write the address of your office.

Car storage location address (Jidousha no Hokanbasho no ichi: 自動車の保管場所の位置)

The address of the car park. When you write this in Japanese, you make full use of google map. This has the Japanese version and if you click on the place, the address in Japanese will be shown in a window on the left side of the map.

Car storage location sticker number

If you used another car before buying the present one, and you acquired the garage certificate by the same location about No.5 and No.6, you write car storage location sticker number which you received when you used the previous car.

If you newly rent a car park and get a garage certificate, you should leave blank.

Police station name and your detailed information (Name, address, phone number, and post code)

In the left side of column No.8, you can find 警察署長殿 (chief of the police station). Before “警察署長殿”, you write the police station name. Generally, the name of the place is used to the police station name. For example, Kojimachi (麹町) is one place name in Hnazomon area in Tokyo. The name of the police station which takes charge of Kojimachi is Kojimachi sho (麹町署). So if you send this form to Kojimachi-sho, you have to write “麹町” before “警察署長殿”。

In the bottom of this application form, there are 3 columns you need to fill in.

使用権原 (Shiyoukengen: the right of user)
自己・他人・共有 (Yourself, other person, sharing)
連絡先 (contact number)
名前(name)
電話(phone number)
新規代替 (car replacement)
車体番号(Vehicle Identification Number)
前車(the previous car)
現車(the present car)

If you buy a new car, you write Vehicle Identification Numbers of both the previous car and the present car.
