The red-bordered section is filled out by a leaser and the upper section is filled out by a renter. So you write your information in the upper columns.
① Car storage location (Hokanbasho no ichi: 保管場所の位置)
Write the address of the car park. Additionally, you write the car park name and the parking space location number, where you park a car.
② and ③ User’s information (Shiyousha: 使用者)
② Postcode, address, phone number
“〒” is postcode. “住所” is the address of your house or apartment. In the right bottom, you write your phone number. If you don’t have a landline, you write your cell phone number here.
③ Your name (Namae: 名前)

④Period of use (Shiyoukikan: 使用期間)
Period of use. In other words, it is a contracting period. When you start renting the car park and when your contract is expired.

A contracting period needs to be valid for at least 1 month. Generally, a one-year or two-year contract is common among renters in Japan. If your contracting term is as short as two months or three months. You should ask a police officer if this contract period has no problem.

If you ask a car park owner to fill out the red bordered section, a car park owner may claim you to pay a fee, which costs you around a couple of thousands of yen.
Depending on the person, this expense is really painful spending. However, this can substitute for a copy of the rental contract of the car park.
But, you need a contract document that describes the name of the contractee, the address of the car park, and the contract period. Then, you can save cost.
