You need the “Notification Card” to have you MN card re-issued by the municipal office
This is “Notification Card” that will be sent to your address if your resident registration has done.

If you want to know your MN number during the application of re-issuancew
If you really want to know your MN number during the applicaiton of re-issuance, you just get your residence certificate from the municipal office. If you request to put your MN number on your resident certificate, the municipal office will issue your resident certificate with your MN number.
Documents and certificates that you need to fix when your MN number changes
If you lost your MN card or your notification card, your personal number wouldn’t change in principle. However, if abuse of your MN number by someone can be assumed and your MN number changes by your request or by the privilege of the head of the municipality, you need to change you MN number (personal number) registered on the following documents and certificates
